    Critique of Integrative Medicine: Integrating Quackery with Science

    I often describe “integrative medicine” as integrating quackery with medicine because that’s what this inadvertently appropriately named branch of medicine in essence does. The reason, as I’ve described time and time again, is to put that quackery on equal footing (or at least apparently equal footing) with science- and evidence-based medicine, a goal that is close to being achieved. Originally known as quackery, the modalities now being “integrated” with medicine then became “complementary and alternative medicine” (CAM), a term that is still often used. But that wasn’t enough. The word “complementary” implies a subordinate position, in which the CAM is not the “real” medicine, the necessary medicine, but is just there as “icing on the cake.” The term “integrative medicine” eliminates that problem and facilitates a narrative in which integrative medicine is the “best of both worlds” (from the perspective of CAM practitioners and advocates). Integrative medicine has become a brand, a marketing term, disguised as a bogus specialty.

    The Evolution and Principles of Functional Medicine

    In the last few decades the emerging sciences of regenerative medicine and molecular medicine helped doctors develop a science based understanding of how to incorporate integrative medical therapies so that they can be allowed to be applied in instances where the applications of traditional pharmaceutical based medications failed. Old became new and from these finding came the original concepts that helped coin the term functional medicine in the early 1990’s.

    Functional medicine is a type of medical science that focuses primarily on interactions between the patient’s physical environment and the patients’ bodily functions such as between the endocrine system, gastrointestinal and immune systems. Practitioners of functional medicine develop individualized treatment plans for the men, women, and children they treat.  This paradigm shift takes healthcare with a disease-centric focus to one of where we look at the relationships of the bodily systems and examine the whole person and not just a few of the isolated physical symptoms.

    As technology advanced over the last 100 years, doctors became more reliant on the use of pharmaceuticals based products and physicians failed to understand the underlying issues and were no longer able apply basic sciences in their practices.  The medical industry became too concerned with about categorizing the names of diseases then implementing the appropriate drug protocols, rather than spending the time needed to better understand the cause of the patient’s imbalances and underlying physiologic dysfunctions.

(IFM.org) Providers spend time listening to the patient and digging deeper into what is causing the patient’s symptoms. As the appointments are often longer than a conventional medicine appointment, the whole person is addressed including, how you feel, what are your goals, what does your diet and exercise look like, how do you think stress is playing a role, and when did you last feel well.

    Through in-depth discussion, thorough intake and exploration the root causes are identified and can then be addressed in a variety of ways starting with diet and lifestyle interventions, use of supplements and herbals, and if necessary referrals or prescription medications. Often patients utilizing a functional medicine practitioner report better sleep, more energy, improved mood, clearer skin and better digestion.

    Addressing Root Causes in Medicine

    Imagine caring for a tree: if the leaves went brown and spotty, you could paint them green to make them look better, or even cut them off. The symptoms will be gone, however, without having addressed the underlying problem, they will return. This is akin to using medications for rapid relief and symptom suppression. The symptoms might disappear for a while, however the underlying problem still exists, while the treatments themselves can often lead to further problems.​

    Consider the management of high cholesterol. Statins reduce cholesterol by inhibiting an enzyme used in its production. The same enzyme is also required to produce CoQ10, a critical antioxidant and cofactor involved in energy production - so synthesis of CoQ10 is reduced which can lead to side effects like fatigue and muscular pain, and an increased risk of developing diabetes mellitus.
